Geriatric Care Management Services

Sterling Care’s geriatric care managers are specialists in issues related to aging and elder care. We provide personalized support to elderly individuals and their families as they deal with the many challenges faced during the aging process. Our unique team approach of geriatric care managers, nurses, social workers, attorneys and accountants allow us to provide all of the resources necessary to assist seniors and their families with the wide variety of issues that need to be addressed in properly caring for an older adult in an efficient and very cost effective manner.

The Sterling Care approach to geriatric care management begins with a comprehensive evaluation and assessment of the client from a holistic body, mind and spirit perspective. We carefully look at their current living and support arrangements and work collaboratively with the client and their family to formulate a short-term and long-term care plan.

Depending on the desires and needs of the client and their family we are then able to provide, as needed, a variety of care management and support services from day-to-day living solutions to more complex medical management and planning in a very cost effective manner. Some of our services include:

Coordinate and monitor service providers
Facilitate communications between patients, physicians, other health care providers and families
Provide RN’s and Home Health Aides to accompany patients medical appointments
Assist with medication management
Provide support and counseling for life transitions
Act as liaison between family members and the older adult
Arrange and oversee homecare support and services
Evaluate long term residential care choices and assist in moving the older adult
Review medical, legal and financial issues

Each case is personally supervised by our Geriatric Care Manager Supervisor Theresa Pizzarello, BSN, MBA with a certificate in Geriatric Care Management.

Sterling Care offers geriatric care management services to customers in the lower Fairfield County Connecticut and includes the cities of Darien, Easton, Fairfield, Greenwich, New Canaan, Norwalk, Southport, Stamford, Weston, Westport and Wilton.
