This partnership creates and monitors an individualized Plan of Care and is ready to provide advice, support and help whenever necessary. A professional Plan of Care for each patient ensures client and family care goals are defined and understood. State-of-the-art communications regularly provide an account of working hours, specific services rendered, physician’s instructions followed and a description of the patient’s progress.
